- Hurts Isaac without actually damaging him, activating any items that would trigger upon taking damage.
Notes[ | ]
- Using Voodoo Pin while having 9 Volt and Habit only charges the item by 1. This is possibly to prevent players from having infinite Voodoo Pin uses.
- It will kill Isaac if he has all his heart containers filled with rotten hearts (possibly a bug).
- It will kill Isaac if used against The Witness unless he also has The Polaroid.

- Habit: No effect.
- Whore Of Babylon: Activates Whore of Babylon's effect for the current room regardless of red hearts available.
- Perfection: Destroys Perfection.
- Polaroid: Activates Polaroid shield regardless of red heart amount.
- Negative: Activates Negative effect regardless of red heart amount.
- Bloody Lust: Every usage increases the character's damage.
- Holy Mantle: Does not remove the shield.

- This item has a counterpart in the Afterbirth† DLC called Dull Razor, which is identical to Voodoo Pin except in appearance, name, and certain interactions.
- Voodoo Pin was one of the few items not imported into Repentance, alongside Book Of Despair, Bowl of Tears, Donkey Jawbone, Knife Piece 3, Menorah, and Stone Bombs.
